Care Team Leader

BDS are currently recruiting for a Care Team Leader to organise and supervise the delivery of care and support services to older adults within an Extra Care Housing Complex in Harpenden

Main duties will include

Managing the rotas of all care staff, matching clients to care workers, ensuring care workers are adequately trained for work assigned to them.

Monitor and report changes in client’s conditions to the Housing Manager, Social Services or named service user representative

Devise individual care plans and packages

Receive referrals from all sectors of the community

Undertake client assessment, risk assessment and client reviews

Attends care plan meetings, reviews or case conferences as required.

Receive all monies collected from clients by home care support workers, ensuring payments are identified and recorded correctly for banking same promptly.

Maintain accurate and confidential records in accordance with the Association’s policy.

Receive referrals from all sectors of the community.

Cover the rotas by matching clients to care workers, ensuring care workers are adequately trained for work assigned to them.

Deliver hands on care when needed.

Undertake supervision meetings with seniors and care workers.

Reconcile timesheets and prepare paperwork for payroll.

The ideal candidate will have a good working knowledge of care services for older people including relevant legislation and minimum care standards. You will also have experience in supervising staff and be a flexible team player. Good IT skills in Microsoft Word, Excel and Outlook are essential. NVQ level 3 in health and social care or equivalent would be an advantage.

Hours are 35 per week and will include some weekends on a rota basis.
